Requirements
- Birth Certificate (Original & Photocopy) SR Prep must be five (5) years old by May 31, 2012.
- Report Cards as of the Third Grading Period (Photocopy)
PROCEDURES
- Submit the requirements to the Registrar’s OfficeFill up the application form.
- Return the accomplished Application Form to the Registrar’s Office.
- Pay the Testing Fee (Php 130.00) at the Finance Office, LRC Bldg., La Purisima St., Zamboanga City
- Return duly accomplished Application Form with the TestingFee Receipt to the Registrar. The schedule for the test will then be set.
