Application for vehicle gate pass for students

APPLICATION FOR VEHICLE GATE PASS FOR STUDENTS

REQUIREMENTS

A. For Salvador Campus (La Purisima St)

1. FOR NEW APPLICANTS

a.       Gate Pass Application Form (available at the University Security Office or downloadable via AdZU website)

b.      Photocopy of student identification card

c.       Photocopy of vehicle registration

d.      Photocopy of enrollment form/official receipt of student enrolled

2. FOR EXISTING GATE PASS HOLDERS (Renewal of Gate Pass)

a.       Gate Pass application Form (available at the University Security Office or downloadable via AdZU website)

b.      Photocopy of enrollment form/official receipt of student enrolled

B. For Kreutz Campus (Tumaga)

A. FOR NEW APPLICANTS

a.       Gate Pass Application Form (available at the University Security Office or downloadable via AdZU website)

b.      1pc 2×2 ID picture of applicant (any background) Photocopy of student identification card

c.       Photocopy of vehicle registration

d.      Photocopy of enrollment form/official receipt of student enrolled

e.      List of authorized driver/s (if any)

B. FOR EXISTING GATE PASS HOLDERS (Renewal of Gate Pass)

a.   Gate Pass application Form (available at the University Security Office or downloadable via AdZU website)

b.   Photocopy of enrollment form/official receipt of student enrolled

PROCEDURE

1.       Submit accomplished Application Form and other requirements to the University Security Office

2.       Secure Payment Slip from USO.

3.       Proceed to the Finance Office for payment.

4.       Present official receipt to USO for validation.

5.       Claim Gate Pass sticker at USO.

Note:

Application for Gate Pass for 4-wheeled vehicles in Salvador Campus is on hold until further notice.
